Kladivem na komára nebo adekvátní léčba od počátku RS?
Abstract
Roztroušená skleróza (RS) představuje nejčastější autoimunitní onemocnění centrálního nervového systému (CNS) a je také nejčastější příčinou invalidity mladých osob. V současné době jsme svědky změn jak v náhledu na průběh choroby, tak především v konceptu léčby. RS je nyní vnímána mnohem více jako „doutnající“ chronický proces, který progreduje ve smyslu axonální ztráty a uzavření zánětu do CNS od samotného počátku nemoci, a to i bez ohledu na přítomnost atak. Adekvátně tomu je nyní snaha posunout vysoce efektivní léčbu (high efficacy treatment, HET) na počátek onemocnění. Pacienty s nepříznivými prognostickými faktory (aktivita RS + infratentoriální/míšní/gadolinium-enhancující léze na magnetické rezonanci) tak ihned v úvodu léčíme nejefektivnějšími léky a nevyužíváme u nich dříve běžný eskalační postup od léků první linie (interferon beta, glatiramer acetát, teriflunomid) postupně výše. Na kazuistice pacientky již 10 let léčené monoklonální protilátkou okrelizumabem (HET) bude demonstrován aktuálně doporučený postup v léčbě aktivní RS od jejího počátku včetně shrnutí rizik této léčby, která se v dlouhodobém horizontu ukazují naštěstí jako málo významná.
